In an effort to uphold the therapeutic freedom and integrity of our practice, we do not accept insurance plans at Collaborative Medical Arts. This enables us to work outside the constraints and requirements of health insurance carriers and ensures us both the time and freedom to provide the comprehensive care needed to address your health concerns.
Fees are due at the time of service. Methods of Payment Include: Credit/Debit Cards, Personal Checks, Cash. If you are unable to pay the set fees, please contact us to find out about possible payment plans, sliding scale and whether or not financial aid is available.
Office visits and medications can be paid through a Health Savings Account or may be reimbursable if you have out of network coverage as part of your insurance plan. We provide receipts with appropriate codes for your insurance company.
Cancellation Policy: Please notify the office of cancellations at least 24 hours prior to your appointment. Cancellations less than then 24 hours prior will incur a charge of 50% of the appointment fee.
Phone Calls: On the weekdays that we are not in the office messages are checked throughout the day, from 8am-6pm. Dr. DeRosa-Lazare is available by cell phone for her established patients after office hours and weekends for urgent calls only. Non urgent messages will not be returned until the next working day.
